This drawing is only open to participants who completed the Massachusetts Medicaid Buy-In Program survey. One entry per participant.

The entry form is completely separate from the survey you just filled out. The survey is anonymous–your name will not be attached to any of the information you provided. We will not add your name to any list or use your personal information in any other way. We will ONLY contact you if you win the drawing. No annoying dinnertime phone calls from us!

The deadline for entering is Dec. 31, 2012.


Note: If you are chosen as the winner, we will need to ask for your BU ID number and report the gift card as income to the Payroll Office.
